Description

Mitochondrial epilepsy (MIT-E) is caused due to mitochondrial dysfunction in patients with mitochondrial diseases (MD) and it is the common manifestation in MD patients. Most epilepsies caused by a mitochondrial disorder begin in childhood and usually in the first 2 years of life. However, seizures can begin within days of birth, or into adolescence or, rarely, into adult life.

Mitochondrial diseases are a heterogeneous group of diseases resulting from energy deficiency and reduced production of adenosine triphosphate (ATP) due to reduced oxidative phosphorylation. Epilepsy is one of the most common manifestations of diseases resulting from mitochondrial dysfunction, especially in children. However, the onset of seizures in MD patients can occur at any age. In some mitochondrial diseases, seizures are part of a syndrome, for example, in Alpers-Huttenlocher syndrome (AHS) and other phenotypes associated with a pathogenic variant in the POLG gene, pyruvate dehydrogenase complex (PDHc) deficiency, myoclonic red fiber epilepsy (MERRF), MELAS, or the syndrome by Leigh.

Till date, more than 300 mutations have been reported, that are known to cause a spectrum of mitochondrial diseases. Failure to produce an adequate amount of ATP or energy is considered the main reason for most of the mitochondrial pathologies, which result in multisystemic disorders. The clinical presentations are extremely severe in high energy demanding tissues, such as skeletal muscle, central nervous system, and heart muscles; however, mitochondrial dysfunction might affect any organ of the body, Additionally, there are changes in mitochondrial DNA/mtDNA(especially with the variants m.3243A> G and m.8344A> G) and in nuclear DNA/nDNA (for example, mutations in POLG) that predispose to the development of epilepsy. Furthermore, seizures are more common in complex I and/or IV of mitochondrial respiratory chain failure than in complex II and III. However, the exact phenotypic manifestation is difficult to predict.

The criteria used to diagnose mitochondrial epilepsy (MIT-E) are those of the diagnostic guidelines provided by Newcastle Mitochondrial Disease (updated January 2013). According to these criteria and the KOL perspective, the diagnosis of epilepsy within mitochondrial disorders requires the combination of clinical evaluation, MRI, and electroencephalogram (EEG). However, diagnosing mitochondrial epilepsy disorders is challenging and mostly recognized in drug-resistant seizures with other non-seizure symptoms. Diagnostic challenges such as genetic testing and biochemistry analyzer, which are expensive and time-consuming, are still a major concern for KOL and think that effective diagnostic tests and guidelines are needed that can steer the patient and healthcare professionals towards the identification of these conditions.

Prevalent cases of mitochondrial epilepsy (MIT-E) in the G7 countries are expected to increase to ~83,000 cases in 2032 with a CAGR of 0.5% for the study period (2018-2032). The United States was estimated to have the highest prevalence of mitochondrial epilepsy (MIT-E) cases in 2032, among G7 countries.

Mitochondrial Epilepsy – Current Market Size & Forecast Trends

Antiepileptic drugs are the mainstay of MIT-E treatment. The goal of epilepsy treatment is to reduce the frequency and intensity of seizures without causing treatment-related side effects or adverse outcomes. Current management of mitochondrial epilepsy relies on antiepileptic drugs (AEDs) with a market share of 100% in 2021 and is expected to decrease by 30% once new drugs will enter the disease space.

Usually, AEDs are effective in controlling seizures in nearly 70% of patients. But there is almost 30% of the population who usually do not respond to anything. This is a refractory group and falls into an unmet need. Mortality rates are highest in intensive care units, where patients often face multiple symptoms along with epilepsy and often experience coma or brain injury due to epilepsy.
